New Patients

At Oklahoma City Indian Clinic (OKCIC), new patients are warmly welcomed, and our dedicated team ensures a friendly and comfortable beginning to their health care journey. With a straightforward registration process and personalized care plans tailored to their unique needs, we prioritize providing a positive and supportive experience for each individual.

Current Patient Information

Adults

To establish a chart at OKCIC, you must fill out a patient registration packet. This can be done one of two ways:

  • Online patient registration
  • Pick up a registration packet at 309 S. Ann Arbor, Oklahoma City, OK 73128.

Once your chart is established, you must schedule an appointment for a new patient physical.

  • You may submit the required documents to the Registration desk at registration@okcic.com
  • Insurance information must be provided for those with existing coverage. Click the button below for more information about insurance.

An appointment must be kept before additional services can be rendered.

Children

Parents must come in to register their child/children. Anyone that is not a parent must provide legal documentation that they have custody, guardianship or foster care.

To establish a chart at OKCIC, you must fill out a patient registration packet. This can be done one of two ways:

  • Patient registration form
  • Pick up a registration packet in Pediatrics located on the 2nd floor of 5208 W. Reno Ave., Oklahoma City, OK 73127
  • Parents will need to complete a registration packet and bring in documentation, including:
    • A CDIB or federally recognized tribal enrollment card (may use parent’s card until their 19th birthday)
    • Certified birth certificate, Social Security card, immunization records and medication profile (list of all prescription and over-the-counter medications)
  • The registration clerk will enter the information and contact the pediatrics clerk to obtain the next available appointment.

An appointment must be kept before additional services can be rendered.

To help ensure the safety and well-being of each child, OKCIC asks you bring your valid Photo ID or Driver’s License to each visit to ensure proper identification of the person accompanying the child at their appointment. We hope you understand this request is to protect each child we serve.

Required Documents

To register as a patient, you must submit original official documents:

  1. Eligibility Documents:
    • CDIB (Certificate of Degree of Indian Blood) card issued by the Bureau of Indian Affairs, or
    • Federally recognized tribal enrollment card, or
    • Letter of Descendancy from a federally recognized tribe.
  2. Social Security Card: All persons must have a Social Security card within one year from date of birth.
  3. Photo Identification: Current driver’s license, State issued ID, passport, or government ID with picture for all persons aged 18 or older. (Tribal enrollment card with a recent photo may be used until a current State ID can be obtained.)
  4. Certified Birth Certificate: Required for all applicants under 18 years of age. Appointed guardians must have court documentation for minor children.
  5. Immunization Record: For persons under 16 years of age.
    Medication Profile: Needed for all applicants. Please provide a list of any medications (prescriptions and over-the-counter) you are currently taking.
  6. Medicaid, Medicare, private or tribal insurance card: If you are receiving Medicaid, Medicare or if you have private/tribal insurance please provide a copy of your card(s) to the registration staff upon each visit. The clinic is required by Federal law to seek and collect payment from any insurance the patient may be eligible to participate in and patients are required to provide this information.

If the name(s) on your CDIB, identification and social security card do not match, you must provide legal verification of name change. This includes marriage license, divorce decree, court papers, etc.

Oklahoma City Indian Clinic reserves the right to make the final determination of eligibility.

Eligibility

A person may be regarded as eligible for services at Oklahoma City Indian Clinic if he/she is of American Indian and/or Alaska Native descent as evidenced by one or more of the following documents:
